Registration Policies:
- All registration fees MUST be included with this form. Registrations will not be processed without payment. Please do not ask to be invoiced.
- All fees must be received by October 21, 2008. Please allow for mail time.
- After October 21, 2008, registrations will only be accepted on-site.
- Cancellations received on or after October 27, 2008 will be refunded minus a $75 processing fee.
- Within 6 days of start date, no refunds will be accepted.
- Only one registrant per form. Registrations are nontransferable. Photocopy blank form if necessary.

Standard Cancellation Policy:
- Cancellations must be received in writing.
- Cancellations received within 7 days of the start date of the conference will receive a refund minus a $75 processing fee.
- No refunds issued for cancellations received within 6 days of the start date of the conference.
